Professional Disclosure Statement

Qualifications

I received a Master of Arts degree in Counseling from Lenoir-Rhyne University in Asheville, North Carolina. I am currently licensed as a Licensed Clinical Mental Health Counselor, NC #13151. Please know that I regularly participate in consultation and supervision with other mental health professionals. This allows me to stay current in my practice and to gain other perspectives on my cases. As such, I may share relevant case summaries with my consultant, however I do my best to remove any identifying information. If you have questions about this process, please don't hesitate to ask. I have experience working with adolescents and adults in both individual and group settings. Areas of treatment have included personal, behavioral, and emotional difficulties, and I specialize in the treatment of relationship issues, personality disorders, addiction, and trauma.

Counseling Services

As a client, it is your right to decide on your personal goals for counseling. While I will be able to assist you in co-creating these goals, you are in control of deciding the issues on which you wish to work. An assessment will be conducted at our first meeting during which we will discuss your presenting issues, symptoms, medical and psychological history, and other information relevant to counseling. The assessment will help determine the type of services that will best suit your needs and guide treatment planning.

Sessions and Billing

Individual sessions are 45 minutes and the fee is $125. I do not accept insurance, however I can prepare a superbill for clients if they wish to seek reimbursement through their insurance company. I recommend contacting your insurance company to see what services qualify for reimbursement. Some health insurance companies will reimburse for counseling services, while some may not. Some reasons that people seek counseling do not qualify for insurance reimbursement.

Use of Diagnosis

When diagnosis is necessary and clinically appropriate, I utilize the Diagnostic and Statistical Manual of the American Psychiatric Association, Fifth Edition (DSM-5) to make clinical diagnoses. If a qualifying diagnosis is appropriate I will discuss the diagnosis with the client before including it in the counseling record. People often seek counseling for reasons that do not qualify for clinical diagnosis. It is important for clients to be aware that insurance companies will generally require a mental health diagnosis before they will agree to reimburse you.

Confidentiality

All of our communication becomes part of the clinical record, which is accessible to you upon request. Records may be obtained by contacting me. I charge $100 per hour, prorated at five minute intervals, for these requests. I am ethically required to maintain confidentiality regarding all of our communications. However, there are a few specific instances in which I would be legally required to break confidentiality. These include any disclosure of current child or dependent elder abuse/neglect, and any plans to harm yourself or others. I must also break confidentiality if I am court-ordered to release your information. Confidentiality will only be breached to ensure the safety of you or someone else. Please be aware that I cannot guarantee confidentiality through email or text and I will not discuss treatment through these methods. I will not engage with clients through any social media platform.

Complaints

Although clients are encouraged to discuss any concerns with me, you may also file a complaint against me with the organization below if you feel I am in violation of the American Counseling Association’s Code of Ethics, found here: https://www.counseling.org/Resources/aca-code-of-ethics.pdf.
